How to get a Medical Marijuana Card

YOUR NEXT STEPS to obtain a patient card:

To participate in the Missouri Medical Marijuana program as an adult (over 18 years old) patient, you must follow these steps:

    Schedule an Appointment with a Physician

    The Physician Certification Form must be completed by an MD or DO in good standing in the state of Missouri. Patients under the age of 18 will also need to complete and submit a Parent/Legal Guardian Consent Form  with their application.

    Register with the Complia application portal through the Missouri Department of Health and Senior Services by clicking here .

    Click on the Patient/Caregiver Application Registration blue box located under the photograph to apply for your identification card. If a “Warning-Exiting Site” pop-up box appears, click “Yes” to proceed to the registry. Only applications submitted through the on-line registry will be accepted.

    Apply for Your Medical Marijuana Card. Once you have registered, an application can be completed by logging into the Complia portal, clicking +Create Application and New Patient Registration. Applications may take up to 30 days for approval.

    Complete Your Application.

    To complete your application, you will need the following:

    • A photograph or scan of the Physician Certification Form.
    • A clear, color photograph of the patient’s face, taken within the prior three months.
    • Proof of Missouri residency, such as a copy of a valid Missouri driver’s license, a Missouri issued ID card, a current Missouri motor vehicle registration, or a recent Missouri utility bill.
    • A legible copy of the qualifying patient’s photo ID issued by a state or federal government entity.
    • Payment for application fees.

    Missouri will only accept online patient registry. Walk-in or mailed applications, documents, and payments will not be accepted and will be returned to the applicant by certified mail.

    Access Your ID Card. Once your application has been approved, you will login to your account with the MMMP Patient Registry. Make sure to save a copy of your patient ID card to your device and print a copy for your records. You are now ready to make a purchase at Organic Remedies or another Missouri licensed dispensary. Your card is valid for one year.

    Make a Purchase

    Be sure to bring a printed or digital copy of your DHSS-issued ID card and a photo ID with you to the dispensary so you can present them to the staff. To participate in the Missouri Medical Marijuana program as a Caregiver:

    Patient Must Schedule an Appointment with a Physician. The Physician Certification Form must be completed by an MD or DO in good standing in the state of Missouri.

    Applying caregivers will need a Patient Authorization Form  and the patient’s license number, which the patient will receive once their application is approved. This number will begin with PAT.

    Patients under the age of 18 will also need to complete and submit a Parent/Legal Guardian Consent Form  with their application.
